Data or life? Ethical obligations to present and future patients.

Each year 10.9 million new cases of cancer are diagnosed worldwide, and 6.7 million people die. The good news is that better drugs are developing faster. We all want to hear about “wonder drugs” and the scientific and medical communities feel the urge (and sometimes the pressure) to provide them. However, some ethical problems might appear in our way to this breakthrough. Before being released into the market, any drug has to undergo a trial in which its benefits are tested and weighed up against its adverse effects. Of course, the desirability of increasing our knowledge in order to improve our health is beyond question. What poses problems is that those individuals who participate in the trials are the means by which we achieve that knowledge. Past experiences, like the Tuskegee Syphilis experiment that took place in the US showed that it’s morally outrageous to trade the life of some individuals for the sake of benefiting society. Nowadays the issue is regulated but moral dilemmas persist.

Why Bioenhancement of Mathematical Ability Is Ethically Important

In a paper just released today, Cohen Kadosh and colleagues (Cohen Kadosh et al., Modulating Neuronal Activity Produces Specific and Long-Lasting Changes in Numerical Competence, Current Biology (2010), doi:10.1016/j.cub.2010.10.007) described how they increased the numerical ability of normal people by applying an electrical current to a part of the skull. So what? Most of us don’t do that much maths after leaving school and manage just fine.

Is Low Libido a Brain Disorder?

Having started to work in the field of neuroethics a couple of years ago, I have become staggered by the misunderstanding of what neuroscience can tell us. The best example is a recent BBC story which goes by the wonderful title “Libido problems: ‘brain not mind‘” .

Is it legitimate to ask opponents of embryonic stem cell therapy whether they support IVF?

In the news this week is the first US officially-sanctioned human trial of embryonic stem cells. A patient with spinal cord injury has received an injection of embryo-derived stem cells.

Predictably, the news has not been received positively by those who are opposed to research with embryonic stem cells.

The development, however, was criticized by those with moral objections to research using the cells because days-old embryos are destroyed to obtain them.

"Geron is helping their stock price, not science and especially not patients," said David Prentice, senior fellow for life sciences at the Family Research Council.

The arguments in favour and against embryonic stem cells have been reviewed and rehearsed ad nauseam. I will not repeat them here.

 

But is it reasonable to ask or demand that those who are opposed to ES cells answer 'the question'. What are your views on IVF?

Is the UK’s HPV vaccination programme unethical and/or unlawful?

A colleague recently emailed me. Her daughter, just turned
12, had come back from school bearing an information leaflet about HPV vaccination
with the Glaxo Cervarix vaccine, and a consent form for the parent to sign.

The consent form nodded inelegantly to Gillick, asserting that ‘[t]he decision to consent or refuse is
legally [the girl’s], as long as she understands the issues in giving consent.’
There was no indication given, in the consent form or the accompanying
literature, as to whether and if so how that understanding would be tested. The
reality is that it won’t be tested at all.

When you prick me do I not cry?

A fascinating study in the Lancet this week has suggested that a very commonly used and simple analgesic in newborn infants may not actually be preventing them from experiencing pain. The study’s authors suggest that this medicine should no longer be used routinely in newborn infants. A headline in the Guardian reads “Newborn babies should not be given sugar as pain relief”. But there are scientific, philosophical and ethical reasons why this conclusion, though possibly correct, is premature.

Ethicists unite: you have nothing to lose but your non-citation

Yesterday Richard Ashcroft, Professor of Bioethics at Queen Mary College, London, wrote in a Facebook update: ‘I am fed up with being asked to come into science/medicine projects, add a bit of ethics fairy dust, usually without getting any share of the pie, just to shut reviewers up. I am not doing it any more. If they think we are important, treat us with respect. Otherwise, get lost.’

Lots of people liked this. So do I. Ethicists have for too long been the invisible but essential backroom boys and girls of biomedicine; patronised by the practitioners of ‘hard’ science; seen as unimaginative but powerful bureaucrats who have to be kept sweet; as despised social scientists who wield rubber stamps made essential by other zeitgeist-dictating social scientists who want to keep their woolly-headed chums in a job; as factotums who don’t deserve to have their names on the papers any more than the temp who does the photocopying. Why is this?

Is there any point in worrying about the tedium of immortality?

Technologies meant to help extend the human lifespan, such as cryonics, or the procedures investigated by gerontologist Aubrey de Grey under the name “Strategies for Engineered Negligible Senescence”, are increasingly an object of discussion, including in the popular press. A recent example of this is John Walsh’s piece in The Independent earlier this month. He is one of several authors who find it worth telling us that they wouldn’t want to live forever, even if they could. At times his article appears to aim merely at being entertaining and polemical, yet his central idea has been put forward by respected philosophers such as Bernard Williams, in his famous essay The Markopulos case: reflections on the tedium of immortality. In short, the idea is that living forever would just be atrociously boring.

 

Should we draw normative conclusions from such pieces about the development and use of life extension technologies, regarding them as superfluous or even downright undesirable? I want to argue for a negative answer to that question.

Greeks and geeks

At Harvard Medical School someone is screaming, reports the Boston Globe. ‘Death!’, he shrieks, ‘Why after all these years have you not appeared?’ He begs for euthanasia, tormented by his pain. Medical students listen to him.

His lines were written by Sophocles, and the students are listening because they have to: it is part of their curriculum.

The remarkable thing is not that Harvard medical students are being marinated in Sophocles, but that the Globe thought it worth reporting. Most medical students in mainstream western universities will get some ethics teaching. Sophocles is just one tool in the teachers’ toolkit. It’s a very effective one, by all accounts, but no different in kind from the lectures and seminars more conventionally deployed. Ethics teaching aims to teach students some problem-solving strategies, and to help them to recognise, evaluate, criticise, cull or cultivate the values that they themselves bring to the wards.
